All of our clinicians are in-network with the insurance plans listed below.

Azalea Mental Health is an in-network provider with CareFirst BlueCross BlueShield and CareFirst BlueChoice networks, including HMO, POS, PPO, and Regional plans across Maryland, Washington, D.C., and Virginia. We also accept most BlueCross BlueShield affiliate plans from other states through the BlueCard network, as long as clients are residents of Maryland, D.C., or Virginia

Azalea Mental Health is an in-network provider with Cigna / Evernorth Behavioral Health.
This includes most Cigna Commercial (Employer-Sponsored) plans, as well as affiliated networks such as Johns Hopkins EHP, when administered through Evernorth Behavioral Health. Our Cigna / Evernorth coverage is open to residents of Maryland, Washington, D.C., Virginia, and Texas.

Azalea Mental Health proudly supports military families and TRICARE members.

We currently work with TRICARE as non-network providers and some of our clinicians are pending in-network status.

TRICARE Select members can see non-network providers. You’re welcome to schedule with us!
Payment is collected at the time of service, and we’ll provide a superbill you can submit to TRICARE for reimbursement. Your reimbursement amount depends on your plan’s non-network benefits.

TRICARE Prime members typically need to see in-network providers.
Since we are currently non-network, you may still be able to see us if your Primary Care Manager provides a referral or authorization stating either:

  • “Network not available,” or

  • Medical necessity due to your perinatal mental health needs.

Session Costs
Your per-session cost is determined by your insurance plan. This may include a copay, coinsurance, or deductible, depending on your specific benefits. We’ll verify your coverage and provide an estimate before your first appointment. Payment (or your insurance copay/coinsurance) is due at the time of service.
A credit card on file is required for all clients. We accept all major credit cards, FSA, and HSA cards.You can view invoices, statements, and superbills anytime in your SimplePractice Client Portal.

Referrals and Authorizations
Some insurance plans require a referral or pre-authorization before starting therapy. It’s your responsibility to check with your insurance company to see if this applies to you.
We’ll help submit claims as an in-network provider, but if a referral or authorization is needed and not in place, your insurance may not pay for sessions. In that case, you would be responsible for the cost.

We are out-of-network for other insurance plans and carriers, including all Medicaid plans. We have opted out of Medicare.

Fee for service rates: 

Individual Psychotherapy

  • Initial Intake Session — $220

  • Follow-Up Session — $205

Couples Therapy

  • Per Session — $250

Sliding Scale & Equity Access

We offer reduced private-pay rates for clients paying out-of-pocket whose household income falls below 500% of the Federal Poverty Level (FPL).To apply, please complete our Sliding Scale & Equity Access Form
Out-of-Network & Superbills

If your therapist is not in-network with your plan, you can still receive care at Azalea by paying out-of-pocket.
You may request a superbill (detailed receipt) through your client portal to submit to your insurance for possible out-of-network reimbursement.

Each superbill includes all information required for reimbursement, including CPT codes, diagnosis, provider NPI, and date of service.
Reimbursement rates vary by insurance plan.

Good Faith Estimate (No Surprises Act)

If you are not using insurance, you have the right to receive a Good Faith Estimate explaining the total expected cost of your care before you receive services.

If your final bill is $400 or more above the estimate, you can dispute it.
